In Person at Client's Home

• At Pilgrimway Lives, you can discover a new type of therapy care that provide therapy at the client's home with personal and better care. With patients coming out of a long stay at the hospital, recovering from surgery or  from an injury, home physical and occupational therapy can be one of the best options to get optimal rehabilitation results.

Our Physical and Occupational therapists provides provide extra support to families and their loved ones to help speed up their recovery. This includes but not limited to 

• Perform initial and ongoing physical therapy evaluations according to APTA standards of practice as well as state standards.
• Develop Plan of Treatment for each patient based on initial and ongoing
• evaluations including short- and long-term goals, collaboration demands of other staff members, and required patient and caregiver involvement.
• Implement Plan of Treatment on a consistent schedule and adjust this schedule and plan according to patient progress.
• Document patient progress according to approved medical documentation and home health care documentation procedures.
• Evaluate mobility, strength, and current level of patient function, while setting goals to improve in these areas that are scaled to patient abilities and realistically attainable.
• Report patient progress to patient, home health care agency and healthcare professionals involved in ongoing treatment/care of patients.
• Develop a plan for patients following completion of their Plan of Treatment with appropriate in-home care services.
• Provides education to patient/family /staff by assessing patient/family educational needs.
• Assist other home health care agency staff members as needed to provide care and create a clean, orderly, and effective environment for rehabilitation and physical therapy.

• Must be at least 18 years old
• Graduate of an accredited Physical or Occupational Therapist Program and active license - Current and in good standing (required depending on your location)
• CPR certification or Basic Life Support certification
• Prior experience preferred, but not required
• Valid driver's license and reliable automobile with insurance where applicable
• Fluent in English and/or French (Bilingual or multilingual individuals are encouraged to apply)
• Be genuinely concerned about helping people and have high moral standards of honesty and integrity!

Applicant must meet or exceed minimum qualifications for each of the following background checks:

• Criminal Background Investigation
• Motor Vehicle Driving Record
• Professional and Personal Reference Checks

Must accept responsibility for learning and adhering to PilgrimWay Lives policies and procedures, be able to function in the home setting with minimal direct supervision, and maintain satisfactory relationships with administrative staff, clients, and family members.

Our care teams are composed of the following:

• Certified Would Care Specialists
• Physical & Occupational Therapists
• Speech Therapists
• Certified Nursing Assistants (Home Health Aides)
• Doctors
• Other Health Professionals (Dentists, pharmacist, etc...)

Pay structure: Base Pay + Profit Sharing + Benefits

Base pay: Depends type of client being cared for, level of skill and overall experience
