In Person at Client's Home or Place of Work

• At Pilgrimway Lives, we pride ourselves in providing top-notch home nursing services to our client's residence or place of work. The ensures privacy and quality care in the comfort of their own home. These private duty nursing services can be for the elderly, children with specific medical conditions or adults who needs out of hospital follow-up care after getting discharged from the hospital. 

This position provides nursing care to individuals in their home residence of place of work. The various responsibilities and duties related to this position included but are not limited to

• Provide home-based care to patients with illnesses, injuries and chronic diseases—often after hospital discharge—under the supervision of an RN
• Assists in planning and providing nursing care with responsibility for the assessment of nursing needs and the initiation of nursing intervention(s) according to all licensing and certification requirements.
• Assesses needs of individuals, identifying obvious symptoms of physical or emotional illness, formulates and executes a plan of care, and evaluates outcomes. Collaborates with multidisciplinary team on the integration of the nursing care plan in the personal plan of care.
• Performs clinical skills such as wound care, IV infusions, diabetes care, medication management and administration, etc, in a competent manner and ensures a clean and safe environment. 
• Management of medications and procedures according to pharmacy and nursing procedure guidelines. 
• Actively documents and participates in the rehabilitation programs of individuals. 
• Ensuring that patients and their families receive optimal care.
• Cue into subtle changes in the patient's status that may require multi-level assessment and intervention to prevent deterioration into life threatening situations. 
• Able to perform CPR.
• Anticipate the patient's needs and be sensitive to the client's nonverbal communication.
• Assess the patient and the family's coping mechanisms and provide/seek support as needed. 
• Serve as the patient's advocate and act as a liaison between the patient, patient's family and other healthcare professionals (functions as the health care coordinator of the assigned client)
• Provides guidance and develops nursing staff when needed to ensure quality activity, care, and treatment for all individuals.
• Provides training and acts as a resource for nursing staff.
• Performs and participates in activities as otherwise assigned by the Nurse Supervisor.
• Assist RN in carrying out patient’s plan of care and teaching patient self-care techniques  
• Educate family on the patient’s plan of care, medication and home safety  
• Provides appropriate pain/symptom management.
• Evaluate patient's response to treatments/medications
• Able to stand, walk, bend, stoop, squat, kneel and reach freely.
• Able to lift, reposition, and transfer patients.

• Must be at least 18 years old
• Graduate of an accredited vocational nursing or nursing program
• Two years of nursing experience preferred
•  Able to provide proof of valid auto liability insurance if assignment(s) include driving own vehicle to transport patient.
• Prior experience preferred, but not required
• License - Current and in good standing (Required depending on your location)
• Valid driver's license and reliable automobile in some case
• Fluent in English and/or French (Bilingual or multilingual individuals are encouraged to apply)

Applicant must meet or exceed minimum qualifications for each of the following background checks:

• Professional and Personal Reference Checks

Must accept responsibility for learning and adhering to PilgrimWay Lives policies and procedures, be able to function in the home setting with minimal direct supervision, and maintain satisfactory relationships with administrative staff, clients, and family members.

Pay structure: Base Pay + Profit Sharing + Benefits

Base pay: Depends type of client being cared for, level of skill and overall experience
